A volunteering certificate signed by an officer should contain:
- Name of the Organization.
- Address for the organization.
- Date the work was performed and number of hours on that date.
- Authorizing Person’s name, signature and contact information.
- Description of the volunteering work performed.
- Full Name of the volunteer (as it is going to show on the PVSA award).
